Program Description:
For more than 40 years, the Greater Philadelphia Chapter has worked to improve the quality of life for people with
ALS by supporting vital ALS research and direct care services to patients and their families, as well as engaging in advocacy at the state and federal levels. We serve more than 1,200 people each year and cover a broad service area that encompasses eastern and central Pennsylvania, central and southern New Jersey, and all of Delaware.

The ALS Association Greater Philadelphia Chapter Mike Kilpatric ALS Transportation Program is available to provide transportation to people with ALS registered in our Chapter.

All persons with ALS utilizing the van must be accompanied by a caregiver.2
Seatbelts must be worn by all passengers at all times while the vehicle is in operation. For your safety, the driver will confirm that seatbelts are secured before operating the vehicle.3
Individuals must be able to exit their home/building without the assistance of the ALS driver.4
Drivers are instructed to wait no more than 15 minutes for the individual at the designated pick-up time. More than a 15-minute delay could result in the driver leaving and the trip being cancelled.5
The driver’s sole responsibility is to operate the vehicle. They will assist with loading the individual into the van and securing the wheelchair; however, they have no medical training and cannot be involved in any other activity except driving.6
The driver must be with the van at all times and cannot accompany the individual into their destination.7
Additional passengers are accommodated based upon the number of additional seats available. We are not able to transport minors.8
Priority is given to patients that need transportation to ALS clinics.9
Weekend, evening, and holiday van service is dependent upon driver and van availability.10
Transportation cannot be used for admission to or discharge from the hospital.
Program Description:
Community Care of the Northeast has served the community since 1985. In the early 1980s, Jean E. Langenbach, RN, BS, realized that the elderly poor and persons with disabilities were in need of services to age safely in their homes.
The agency began operations in Holmesburg Methodist Church, moving first to a Presbyterian church and then to 8015 Frankford Avenue before taking residence in 2008 at our current site on Welsh Road. Remaining faith-based and considering the whole person (body, mind, spirit), Community Care of the Northeast provides services without regard to religious affiliation. The intent is to enable the vulnerable elderly who are at significant risk of institutionalization to maintain their independence and quality of life for as long as possible.
Transportation: Wheels for Independence is a transportation program that operates Monday through Friday 8 am — 4 pm. We serve seniors and individuals with disabilities who can ambulate independently, with or without the assistance of a cane or walker.

Clients in the Wheels for Independence program pay a modest annual fee for unlimited rides.2
A donation of $11 per ride is offered by the passenger to the driver.3

Program Description:
In-home support programs are increasingly important for seniors today. As our loved ones choose to age in home, surrounded by the comfortable settings to which they are accustomed, providing support is increasingly important.
Our In-Home Support Program in South Philadelphia provides short term assistance if you are temporarily or situationally homebound due to a brief illness, injury, surgery or other circ*mstances. We offer light housekeeping, personal care, senior companions, and minor home repairs. Access to long term home delivered meals is available.
This program is offered in partnership with the Philadelphia Corporation for Aging. In order to qualify you must be 60 years or older. There is no income eligibility requirement. Transportation assistance is also available to those 65 and older.

In 1902, Lutheran Settlement House (LSH) was founded as part of the “Lutheran Social Mission Society of Philadelphia,” an urban ministry of the Lutheran Church in southeastern Pennsylvania.

Program Description:
The churches in the Allegheny Valley area desire to witness their unity in Jesus Christ as Lord and Savior through the Allegheny Valley Association of Churches.
The Allegheny Valley Association of Churches provides an ecumenical agency for cooperation in evangelism, mission, Christian education, worship and social concerns.
The Association is made up of both clergy and lay people from 54 member churches in the Allegheny Valley area. The Association of Churches reaches out to the community in a number of ways, all demonstrating that the love of God does make a difference in people’s lives.
Faith-In-Action:
This program provides volunteers to the elderly and disabled members of the Allegheny Valley community. The basic purpose is to provide “neighborly acts of kindness” to help ease the isolation of the frail and elderly. Program Description:
BVRS is a private, non-profit agency that believes in independence through rehabilitation. Our roots in the Pittsburgh community are deep. In 1910, the Pittsburgh Association for the Adult Blind (later known as Pittsburgh Blind Association) was formed to provide employment to people with vision loss.
In 1959 the Greater Pittsburgh Guild for the Blind of Bridgeville was created to help people with vision loss develop independence skills.
Those organizations merged in 1997, and for a short time the new agency was called Pittsburgh Vision Services. In 2005, the Board of Directors gave the agency a new name—Blind & Vision Rehabilitation Services of Pittsburgh—to better reflect what the agency does and those whom it serves.
Special Services are funded by the Pennsylvania Association for the Blind (PAB).
Transportation
This free door-to-door service provides rides to any type of medical appointment or essential service need, such as obtaining a photo ID. Individuals must live in Allegheny County and appointments must be made in advance.
Support Services
These services include grocery and personal needs shopping with the individual, assisting with reading and filling out forms, picking up prescriptions, minor household repairs, adjusting clocks and reading instructions.

Community LIFE is an all-inclusive care program tailored to the specific needs of seniors. Participants in Community LIFE receive comprehensive individual care in our Health and Wellness Centers, in-home services, and convenient transportation.

Program Description:
The Flivver is a 13-passenger bus that serves Forest Hills proper and is available for use by Senior Citizens and those with special needs.
It is sponsored by Forest Hills Community Service, a non profit organization, which may be contacted via the borough office at 412-351-7330.
The Flivver is staffed by volunteers (drivers must be 25 years of age or older with a current driver’s license) and new drivers are always needed. The Flivver service relies on contributions to offset costs (send contributions in care of Forest Hills Community Service, 2067 Ardmore Boulevard, Pittsburgh, PA 15221).

For twenty-one years, Heritage Community Transportation has been privileged to provide 1.5 million rides on our fixed route transit service.
Our three routes – covering Monroeville, McKeesport and East Pittsburgh – run through sixteen communities and provide our riders with access to work, healthcare and shopping for only 25 cents a ride.

AgeWell Rides: AgeWell Rides keeps you active and independent, can ease the depression of giving up driving, and allows you to be less dependent on family members for transportation.

Program Description:
The Monroeville Senior Citizen Center, built in 1980, has become the hub of activity for Monroeville and Pitcairn seniors. The center provides daily activities, services, and programs for adults 50 years of age and older.
The center also serves as an informational bureau for seniors often referring them to other agencies. The senior center is owned by the Municipality of Monroeville and operated by the Monroeville Council of Senior Citizens, incorporated under an agreement with the municipality.
The center has over 5,000 members. On average the center has approximately 6,000 visits per month with an average of 72,000 visits per year.
The center offers a variety of weekly and monthly activities: Social clubs, Fitness clubs, Wii bowling league, shuffleboard leagues, card groups, Mah Jongg group, dominos group, health and wellness programs, arts craft clubs, monthly craft classes, monthly bingo and luncheon, movies days, instructional classes, computer classes, trips and tours, and monthly special events.
Services also offered to seniors include health services, health screenings, counseling services, income tax assistance and drivers training classes. Free bus transportation is offered to Monroeville and Pitcairn seniors for regularly scheduled club meetings, special events, & shopping trips.

North Hills Community Outreach is a community- and interfaith-based organization addressing the needs of people in crisis, hardship and poverty.
In Service of Seniors: North
This program helps older adults in northern Allegheny County age 60 and older live healthy and safe with dignity in their homes through volunteer-based practical supports. There are NO income restrictions and services are free.
Before services can begin, a North Hills Community Outreach coordinator must conduct an in-home visit. Once that is completed, we ask for one week’s notice on all transportation or other service requests.
Services are provided by trained, qualified, supportive volunteers who are required to have criminal background and personal reference checks.

This volunteer-staffed shuttle provides door-to-door shuttle service each weekday from 10 a.m. to 4 p.m.2
Seniors older than 60 can call after 9:45 a.m. on the day they need a ride.3
One shuttle provides transportation every day for seniors living in Blawnox, Sharpsburg, Aspinwall, Fox Chapel and O’Hara along or near Freeport Road.4
A second shuttle services seniors in Etna, Millvale, lower Shaler, Sharpsburg, Tarentum, Natrona Heights, Cheswick, Brackenridge, Oakmont, and Verona on various days of the week and will pick up seniors at designated high rises to take them to UPMC appointments.5
Transportation is provided at no charge to those 60 and older.

Program Description:
The Allegheny County Port Authority is now offering Senior Citizen ConnectCards. Senior Citizens, age 65 or older, who present this ConnectCard at the time of fare payment RIDE FREE. Medicare cards are no longer accepted.
This card allows seniors to ride other public transit systems, for free, across the entire Commonwealth of Pennsylvania, not just Allegheny County. Simply tap your card at the fare box and be on your way. Port Authority will continue to accept blue or yellow PA Senior Transit ID cards indefinitely.

Program Description:
Faith in Action, a free community outreach program of the Sewickley YMCA, provides the elderly in our service communities with volunteer drivers to
help them remain independent and a vital part of their community by providing rides to doctor appointments and medical treatments. Volunteers are vital to helping folks conduct errands that keep them living independently in their homes. Faith in Action also provides information and referrals for additional life-sustaining services in the community.
Since the start of the pandemic, Faith in Action volunteers have checked in on our elderly care receivers with phone calls and visits, delivered prescriptions and groceries, and delivered weekly and bi-weekly food boxes to families and individuals struggling with food insecurity due to sudden loss of jobs and other economic hardships caused by COVID-19.

PALS (People Able to Lend Support) is a volunteer service credit program that provides non-medical help to eligible Highmark Health Plan Medicare members.
Over the past 20 years, PALS has helped people get the non-medical assistance they need. In that time, PALS has earned 7 national, local, and statewide awards. But, more important than all of that, are the lifetimes of relationships that we’ve fostered.

LANtaVan is a door-to-door, shared ride (or paratransit) service, available to riders in Lehigh and Northampton Counties who are registered and certified as eligible for sponsorship under various transportation funding programs.
The service is public transportation and is shared ride in nature, that is, no one passenger has exclusive use of the vehicle.

- People with a valid Medical Assistance (M.A) ACCESS Card are eligible to use LANtaVan service to go to M.A. reimbursable appointments (doctors, medical tests and treatment, dialysis, etc.) if they meet the PA Department of Welfare’s program requirements.

Program Description:
Community Resources for Independence (CRI) services began in 1978 under Erie Independence House (EIH) as a 12 unit apartment complex providing attendant care and independent living services to people with disabilities.
In 1980, Timothy Finegan became the Executive Director of EIH. This also marked the year the Federal Center for Independent Living (CIL) grant was awarded to EIH. In 1990, CRI was established and incorporated to become the managing agent for EIH and their CIL services. EIH continued managing their apartment complexes. In 1997, due to federal regulations, CRI separated from EIH to continue providing CIL and Attendant Care services to people with disabilities.
Transportation Services
CRI maintains a fleet of wheelchair accessible vans which we use to provide transportation to consumers in need in Erie County.

Program Description:
Millcreek Community Hospital is an osteopathic acute care hospital offering comprehensive inpatient and outpatient services and 24-hour emergency care in Erie, Pennsylvania.
Transportation Service:
Millcreek Community Hospital offers a complimentary non-emergency wheelchair van transport service to hospital patients who need transportation to home, nursing homes, rehabilitation centers, or a MCH physician’s office.
In addition, the Millcreek Community Hospital van can provide transportation to patients from their home back to the hospital for tests, procedures, or physical therapy. The Millcreek Community Hospital van is also utilized for MCH wound care clinic patients.

Program Description:
The City of Reading and the County of Berks purchased the failing Reading Bus Company in 1973.
This led to the creation of the Berks Area Reading Transportation Authority. BARTA began operations on October 8, 1973.
PA Senior Citizens Free Ride Program:
If you are 65 years of age or older, you may ride free on the BARTA Fixed Route bus system ALL DAY / EVERY DAY!

Program Description:
The Medical Assistance Transportation Program (MATP) is a transportation service available to Medical Assistance consumers in Berks County.
MATP is funded by the Pennsylvania Department of Public Welfare.
In Berks County, MATP is run by the Berks Area Regional Transportation Authority (BARTA) Special Services Division. Our program offers transportation or mileage reimbursem*nt to help you get to medical care or services from a Medical Assistance provider. We are required to provide you with the least expensive, most appropriate transportation service available that will meet your needs.
You can use MATP services to go to medical appointments or to get to any service that Medical Assistance pays for. These medical services include therapies, test, dental visits, trips to the pharmacy to get prescriptions, mental health treatment, drug & alcohol treatment, and trips to medical equipment suppliers.

Depending on where you are going, what your needs are, and the costs involved, we could provide you with transportation in one of the following ways:

day of the appointment,
• Shared-ride service with BARTA or our sub-contractor.

Mileage Reimbursem*nt
If you have a car, or if you know someone else who has a car and he/she can take you to your appointment, we will provide you with mileage reimbursem*nt if it is the least costly, most appropriate service available. We will reimburse you at the rate of twelve cents ($.12) per mile. We will also reimburse you for your actual parking and tolls if you provide dated receipts showing how much you paid.

Program Description:
Without a system of public transportation in place, the greater Boyertown area can be difficult to navigate if you don't have a car or the ability to drive.
Fortunately most people do have family or friends they can count on to get them to the grocery store, pharmacy and to medical appointments. However, some people do not have anyone to help, and for these individuals Multi-Service volunteers can provide rides to medical appointments.
Our volunteer transportation program is a needs-based program and not provided solely for convenience.

- Please contact one of our Case Managers who will meet with you to determine if you are eligible.
(Once an individual is approved by a Case Manager for Volunteer Transportation, he/she can then contact our Volunteer Coordinator to schedule a ride. Every attempt will be made to accommodate a request, but transportation is dependent entirely upon availability of a volunteer and is never guaranteed. )

Funded by a grant from the Corporation for National and Community Service, Diakon offers the RSVP program in Berks, Pike and Wayne counties of Pennsylvania.
While RSVP services vary across the country, our emphasis in Berks, Pike and Wayne counties is on:
- Packaging and delivering Meals on Wheels to our homebound neighbors.
- Providing assistance at your community food pantry.
Transporting clients who no longer drive to medical appointments or to shop for groceries. In Berks County, this aspect of RSVP continues the Diakonprogram previously known as Volunteers Serving Seniors.
Offering service in a local senior community center.
Becoming a member of the local APPRISE program team counseling clients on Medicare and health-insurance-related matters.
We are particularly focused on supporting adults 60 years of age and older to remain in their current living situation and to live independently as long as possible. Please note that additional volunteer opportunities may be available.

For over 92 years the Vision Resource Center of Berks County has proudly provided services to prevent blindness and make it possible for visually impaired Berks County residents to lead independent and productive lives within their community.
The agency serves over 600 blind and visually impaired Berks County residents. The Vision Resource Center of Berks County is a 501 (c) 3, non-profit social service agency that was founded in 1929 by the Reading Lions Club.

Senior Citizens age 65 or older who present a Pennsylvania Senior Citizen ID Card or a Senior ConnectCard at the time of fare payment ride free. Effective January 1, 2020, Medicare cards will not be accepted.
The Senior Citizen ConnectCard allows seniors to ride Port Authority and other public transit vehicles for free across the Commonwealth of Pennsylvania.
Our Senior Citizen ConnectCard works just like a ConnectCard but does not need to be loaded with stored value or a pass. Simply tap your card at the fare box and be on your way.
Port Authority will continue to accept blue or yellow PA Senior Transit ID cards indefinitely.
